<p>A Team Charter is the official document from the team sponsor that empowers the team to act. It is a written document describing the mission of the team and how this mission is to be accomplished.<br />

<p>The Team Charter is one of the most under-used and under-valued tools available to sponsors, team leaders, and facilitators for helping a team succeed.But it is often overlooked because it is time consuming to develop. To write a clear, concise, and inclusive Team Charter requires great forethought and effort.</p>

<p>Often the sponsor is in such a rush to start the teaming process that they skip the basic step of filling out a Team Charter that clearly defines the goals and objectives to be achieved. When a team is started without a formal Team Charter many meetings are often wasted trying to figure <strong>“why we are here, what we should do, and when it should be done.”</strong> The team spends a considerable amount of time and energy trying to second guess what the sponsor really wanted when the team was formed. This is a loss of valuable time and talent that could be avoided by designing a clear mission statement prior to the team’s formation. That&#8217;s why a sponsor must take the time to fill out a Team Charter.</p>
<p><span style="text-decoration: underline;"><strong>The Team Charter is an official work contract.</strong></span> This document delineates the strategic goals, boundaries, measures of success, constraints/limits, and available resources. The Team Charter provides a framework for ongoing discussions between the team and its sponsor with regard to the team’s direction and progress.</p>
<p>It is an iterative process until a base line acceptance is established. It must be reviewed on a regular basis by the sponsor, the team leader, facilitator, and team members to ensure that it is reflective of what the team is doing or will be doing in the future.</p>
<p>The following example of the use of a <span style="text-decoration: underline;"><strong>Team Charter in a Public Health Department</strong></span> is an excellent adaptation of the concepts in the original team charter article to provide a guidance document to beginning Process Action Teams.</p>

<strong>Hepatitis B (Hep A, Hep B);</strong> seasonal influenza; polio (IPV); pneuococcal disease (PCV); and rotavirus (RV). There are two schedules: one covers birth to 6 years and the other covers from age 7 to 18 years. There is also a &#8220;catch-up schedule&#8221; to help those who need to get back on track.</p>

<p style="text-align: left"><strong>Dr Amanda Dempsey</strong>, assistant professor of pediatrics and communicable diseases at the University of Michigan in Ann Arbor, and colleagues, write that in the US, more and more parents appear to be departing from the CDC&#8217;s vaccination schedule. So they decided to see how widespread this was nationwide and also investigate the extent to which those that are currently following the recommended schedule might deviate from it in the future.</p>
<p style="text-align: left">To do this they carried out an internet-based survey that took a single &#8220;snapshot&#8221; of a nationally representative sample of parents of children aged from 6 months to 6 years. Using statistical tools, they then looked at links between various demographic characteristics of the respondents, factors that might influence their decisions, and their use of an alternative vaccination schedule.</p>
<p style="text-align: left"><strong>748 parents took part (response rate of 61%) in the survey</strong>. The results showed that:</p>
<p style="text-align: left">Of those who followed an alternative schedule, just over half (53%) refused only certain vaccines, and/or delayed them until their child was older (55%); while 17% reported refusing all vaccines.</p>
<p style="text-align: left">Non-black parents, and those whose child did not have a regular health care provider, were the only factors to be significantly linked to following an alternative schedule.</p>
<p style="text-align: left">28% of the parents who were following the recommended schedule thought that delaying vaccine doses was safer than the schedule they were following.</p>
<p style="text-align: left">And 28% disagreed that the recommended schedule was the best one.</p>

<p>the disease, enabling the researchers to examine total, unprocessed and processed red meat, poultry and egg intake in relation to risk of lethal prostate cancer.</p>
<p>Using a <strong>&#8220;case only&#8221;</strong> approach, they also analyzed survival rates in relation to the post-diagnosis consumption of these same foods in <strong>3,127</strong> of men who were initially diagnosed with non-metastatic (ie localized, not yet spread to other organs) prostate cancer during the follow up.</p>
<p>The researchers report that for the risk analysis, they noted there were 199 observed events over 306,715 person years <strong>(when you multiply the number of people observed by the number of years each was followed for and compare this to the number of events you are interested in observing, you get a sense of how populated the &#8220;map&#8221; of available data points for analysis is).</strong></p>

<p><strong>The aim of 3D kaizen activity</strong> must be not only to remove the 3D elements but to create environments that respect and draw out human potential for doing good. At minimum this means positively replacing the 3D conditions with clean and uncorrupted processes, suitably challenging work that fulfills our sense of purpose, and safety within the workplace that goes beyond physical and emotional harm and also considers security of employment as well as long-term environmental sustainability. Kaizen activity becomes dull and hollow when we lose sight of the fact that we are pursuing the ideal, pursuing perfection. We must work to replace 3D working conditions with those that are spotless, stimulating and secure.</p>
